1) 90 day warranty period (beginning on original pruchase date).
2) Allows up to 2 additional exchanges per eye within 90 days.
3) Full credit on Basic Lens charges upon patient cancellation (all lenses must be returned within 90 days).
4) Warranty does not cover additional charges.
5) Warranty must be purchased with original order to receive full credit.
6) Cannot be used for broken or lost lenses.

SPHERICAL UPGRADE WARRANTY

1) Can only be purchased within 60 days of original purchase.
2) 90 day warranty period (beginning on original pruchase date).
3) Allows up to 2 additional exchanges per eye within 90 days.
4) 50% credit on Basic Lens charges upon patient cancellation (all lenses must be returned).
5) Warranty does not cover additional charges.
6) Cannot be used for broken or lost lenses.

TORIC AND CONE WARRANTY

One exchange at 100% credit. Any additional exchanges are at 50% credit, within 90 days. $25.00 cancellation fee per lens if all lenses are returned within 90 days.

BIFOCAL WARRANTY

Essential Bifocal and Essential-Xtra
One exchange at 100% credit. Any additional exchanges are at 60% credit, within 90 days. $20.00 cancellation fee per Blanchard Contact Lens lens if all lenses are returned within 90 days.

Lifestyle Dia / Marquis Low Add / Marquis High Add
Two free refits within 90 days. $20.00 cancellation fee per lens, if all lenses are returned within 90 days.

Boston Multivision
Two free refits within 90 days. $20.00 cancellation fee per lens, if all lenses are returned within 90 days.

Tangent Streak Two free refits per lens. 50% credit for cancellation within 90 days.

Amfocal Two free refits within 90 days. $20.00 cancellation fee per lens, if all lenses are returned within 90 days.

Unilens RGP Aspheric Plus
One refit within 90 days. $20.00 cancellation fee per lens, if all lenses are returned within 90 days.

ROSE K WARRANTY

*One exchange at 100% credit. Any additional exchanges are at 75% credit, within 90 days. $20.00 cancellation fee per lens if all lenses are returned within 90 days.

DYNA Z WARRANTY

One exchange at 100% credit. Any additional exchanges are at 75% credit, within 90 days. $20.00 cancellation fee per lens if all lenses are returned within 90 days.

MENICON Z WARRANTY

*One exchange per eye within 90 days of the original order. Lenses must be returned with a copy of the original invoice within 90 days of original invoice date. Credit will be issued on basic fee less a $5.00 per lens recovery fee. Add on charges are not subject to credit.
