Anti-Reflective Warranty Endura, Puresite Preference AR, Crizal, Crizal Alize, and Teflon lenses are
warranted for the life of the Rx (up to two years). Diversified will replace
the lenses and coating providing the prescription remains the same. Warranty
does not apply for damage due to improper care. Doctor remakes will be made at
a cost of 50% if redone within 90 days. Mirror Coating Prescription mirror coated lenses are warranted for up to one year against coating defects. Warranty does not apply for damage due to improper care. To qualify, please return the lenses with a copy of the original invoice. Scratch Resistant Coating All scratch resistant coated lenses are warranted against scratches for one year
from the original invoice date limited to one pair replacement at no charge.
Scratches through normal use will be covered, however, warranty does not apply
for scratches due to improper care.Progressive Lens Replacement Policy Doctor’s Remakes or Telephone Orders This policy will be honored for 90 days from the original billing date, one time per full-charge Rx. When ordering replacement lenses, you must note the words “Doctor or Telephone Error” and supply the original invoice number and date. If the original lenses are not returned when the new lenses are started, the new lenses will be treated as a new order and invoiced at full price. The original lenses must be returned with the replacement invoice at which time a credit of 50% for cut and edge lens will be issued or 20% for uncut lenses. If the exchanged or remade product is at a reduced cost from the original product purchased, then the returned amount credited will be calculated on the less expensive product using the above-mentioned percentages. Rx Changes or Cancellations If the lenses are in process and cannot be used on the Rx, a 50% charge for the lenses will be made. If the lenses are not yet in process, no charge will be made. Drill Mount Warranties All drill mount eyewear processed with Trivex will be warranted for one year. Drill mounts processed with polycarbonate lenses will have a 180-day warranty. High Index materials will have a 90-day warranty if used in drill mount frames. Patient’s Own Frame or Lenses Due to our inability to inspect frame conditions and lens quality before the merchandise is received, we cannot accept responsibility or liability of lenses or frames supplied by patients if damage occurs during processes sing or assembly. Lenses Only Accurate sizing can only be assured when the frame is enclosed. If this is not possible, please provide an accurate circumference (c-size) measurement of the patient’s old lenses. Soldering Frames Soldering will be done at the patient’s risk. Tinting and re-edging of patient’s old lenses. Any processing of previously manufactured lenses will be done at the patient’s risk. Segment height The seg height is defined as the vertical distance from the top of the segment to the bottom-most part of the lens. Executive PD’s It is our policy to give the distance PD on executives since this is the position most used by the patient. If you prefer a near PD instead, please circle the near on your Rx form or advise our personnel when placing the order by phone or electronically. Base Curve Our computer will select the base curve most optically correct for the prescription unless otherwise specified. Requested base curves that vary by more than 2 diopters from standard will be charged at nonstandard base curve rate. Shipping & Insurance We will make every effort to ensure prompt, safe delivery of all eyewear shipped by us. We cannot, be responsible for any products shipped to us, that we do not receive, or that we receive damaged. Handling Charge All Rx work will be shipped by UPS, courier delivery or first class mail.
