How do I begin using my Diversified Account?
Your vendors need to convert your billing so that your purchases are billed through Diversified. You can complete an account conversion form and we will switch the accounts for you or you can do this yourself by contacting each vendor directly.
What is an account conversion form?
This is a document that makes it easy for you to switch your vendors to Diversified. It lists every vendor available in our group. All you need to do is place a check mark or write in your account number next to every vendor you would like to bill through Diversified. If you complete this document and fax it to us then we can contact your requested vendors and make the changes for you.
Do I have to complete an account conversion form to change the billing of my accounts?
No. You can contact the vendors directly and request the change. Simply call each vendor and ask them to set up your account to bill through Diversified Ophthalmics Buying Group. They will ask for your Diversified account number and then they will contact Diversified to verify membership. Some vendors do require more than a phone request.
